Jasmine Stefanovic (formerly Jasmine Yarbrough) has shared a touching tribute dedicated to her late grandmother, who has passed away at 96-years-old.

The shoe designer took to her personal Instagram to upload an emotional poem about grandmothers entitled Nana’s Love* by Sunburn Poet Leanne Laydon.

Along with the beautiful words, Jasmine’s post featured two pictures of herself with her Nana – one from when she was a child and another more recent snap.

“The days may dance too swiftly as your Quickstep becomes a Walt, but your voice still sings with angels Sunday mornings, when it matters most,” begins the emotional poem.

“Take your afternoon tea and chocolate and come down memory lane, hold my hand on Burleigh beach where my childhood days remain.

“With the softest touch of Nana’s love guiding as I’ve grown, you shone brighter than any star in a class, all on your own. I learnt the art of being a lady. The value of grace, and being kind,” it continues.

“You always matched your necklace and dress, these little things come to mind. And fill my cup with admiration for those values instilled along the way because I wouldn’t be the woman I am, without Nana’s love, today.”

Last month Jasmine travelled from her Sydney home to Brisbane to spend quality time beside her grandmother.

Her husband, Karl Stefanovic, confirmed on the Today Show that his wife and daughter, Harper, had received an exemption from Queensland Health to hotel quarantine in the state.

The breakfast TV host added that Jasmine’s grandmother was “really, really, really, sick.”

“Nana ~ with you now and forever 🤍♾.”

In September, the 37-year-old shared a picture holding her Nana’s hand on Instagram.

She captioned the emotional post, “Nana ~ with you now and forever 🤍♾.”

Ever a caring husband, Karl supported his wife in the comment section by writing, “She’s such a beauty. 💕.”
